Notes:	NO LATE ADMITTANCE: Class starts @ 9:00 a.m. All forms handed out at the session must be completed in full and turned in prior to leaving the workshop. *Anyone who submits Incomplete Forms, will not receive credit for workshop attendance and will not be allowed to work or volunteer with Children & Youth. NO EXCEPTIONS! Please do not bring children or youth 15 & under. They will not be allowed to stay. No Exceptions! All YOUTH (16-17) must bring a signed parental consent form for admittance.
